- Shawano-Menominee Counties Community Health Survey The Shawano-Menominee Healthy Communities (SMHC) is a community health coalition working to improve the lives of the people in the two counties. As part of this year’s Community Health Assessment process, we are looking for members of the communities (Shawano/Menominee Counties) to help paint the current picture of our local community, through your own experiences and responses by completing this assessment.
